Position: Care Coordinator LPN

MD Parker Home Services offers private duty, non-medical home care services to elderly and disabled clients in the comfort of their own residence. We work as a team to achieve high standard in-home care.
To be successful in this position, candidates must have a high sense of urgency to stay on task with a variety of priorities. In addition, successful candidates will have excellent communication skills, focus on delivering exceptional client services engagement, and home care industry experience.
Responsibilities:
- Conduct New Client Care Assessment and Care Plan Development
- Respond to questions regarding client and caregivers’ schedules
- Notifies clients with basic information on caregivers, service start date and times caregivers will be providing care
- Develops and documents Client Care Plans
- Familiarize caregiver with client information, direction, time and basic introduction of clients need and diagnoses
- Completes Assessment document
- Document skills and attributes of caregivers to ensure strong matching to clients
- Update the office team on scheduling issues and HR recruitment needs
- Maintain computer schedules and ensure timely data entry for clients and caregivers
- Maintain confidentiality of all information pertaining to employees and clients
- Perform other functions as deemed appropriate by the management team
- Reports directly to the Executive Director
Required Skills and Experience
- Associates Degree or higher
- Proficient in MSOffice (Outlook, Word, Excel, and PowerPoint)
- Flexibility and good time management skills A MUST
- 3+ years of home care or Health care experience strongly preferred
- 1 to 2 years’ experience as a Scheduling Manager and/or managing customer service within a healthcare setting; knowledge of HIPAA and healthcare office regulations.
- Excellent organization, planning, and project management skills.
- Valid driver’s license; valid state required proof of auto insurance.
- Strong phone skills
- Strong Problem-Solving skills
- Ability to multi-task and work well under time pressure
- Positive, friendly, and approachable attitude and ability to work well in a close, team environment
- On-call required – rotating (as needed)
- Must pass a drug screen and criminal background check
